HAWKINS MIDDLE SCHOOL NEW STUDENT REGISTRATION The Parent/Guardian of the student must be present when the student enrolls in school. At the time of enrollment, new students must present the following: Mississippi Certificate of Immunization as required by law. These are available from the Scott County Health Department in Forest for students transferring from other states or countries. Students transferring from in-state public schools must furnish a certificate of immunization if none is sent with the official transcript (school records) from previous school. Certified Copy of Birth Certificate. Social Security Number. Transcript or Report Card from previous school so that proper grade placement is made. All grade placements are based on placement made by previous school and are contingent upon receiving official transcript from previous school. Proof of Parent(s)/Guardian(s)' Residency in the Forest Municipal School District.
