Registration/Admission Requirements

Forest High School has a long tradition of being Superior in Academics and Extra-Curriculum Activities.



A student attempting to enroll in Forest High school must meet ALL of the following requirements:

1. The student must be accompanied by a parent or legal guardian at the time of enrollment.

2. The parent or legal guardian must provide legal documentation which verifies that the parent or guardian is a legal resident of the Forest Municipal School District. Refer to the residence checklist form for a specific listing of the types of documentation.

3. Students residing with a legal guardian must provide written documentation that a legal guardianship actually exists. Under Mississippi law, guardianships established for the purpose of school attendance or enrollment are not permitted.

4. The student must present a withdrawal form issued by the student's previous school. The student must have withdrawn in good standing (not have been suspended, expelled, owe fines or fees. Refer to #7 below regarding suspensions and expulsions.) A student in grade 9 must provide written evidence that he has been promoted to the 9th grade. A student in grades 10-12 must present an unofficial copy of his transcript of credits.

5. The student must present a birth certificate and a social security number.

6. The student must submit a Mississippi immunization certificate. Students transferring from a Mississippi public school will be admitted provisionally pending receipt of the student's immunization certificate from the previous school. Immunization certificates issued by another state cannot be accepted.

7. The parent/guardian must sign a statement of disciplinary status.*

*Mississippi Code 37-15-9: A child or parent must now indicate on the registration form or by means of a statement of disciplinary status if the child has been expelled from any public or private school or is currently a party to an expulsion proceeding. ....The district may deny admission until the superintendent or his designee as reviewed the record and determined that the child has participated in successful rehabilitative efforts, including progress in an alternative or similar program.

If the child was expelled or is a party to an expulsion for an act involving violence, weapons, alcohol, illegal drugs or other activity that may result in expulsion, the district is not required to grant admission or enrollment before one (1) year after the expulsion.

The parent and student are responsible for providing the documents required for enrollment. Forest High School is NOT responsible for obtaining the documents listed in items 2-6 above.
